The Breast Health Patient Navigator coordinates breast care services and serves as point of contact for patients, care givers and medical providers. Their primary responsibility is guiding breast health patients through the health care system by assisting with access, scheduling, education and follow up. This individual assists in the development and implementation of a comprehensive breast care service program across the organization. In addition, this individual works with community resources and partners to assist patients in navigating the health care continuum.

Coordinate breast care services across the health care system.
Serve as point of contact for patients, care givers and medical providers in coordination of care.
Assist in the development and implementation of a comprehensive breast care navigation program for the organization.
Use clinical skills and expertise in the area of breast disease, breast cancer treatments and psychosocial impact of disease and recovery.
Effectively and efficiently coordinate, implement and monitor the care management process and treatment plan for patients.
Communicate openly and empathetically with patients and families.
Provide education and support for the patient and their families.
Develop and maintain a learning library for patients which includes language specific materials.
Work collaboratively with the care team, including outside imaging centers, to develop and implement the patient care plan.
Monitor patient outcomes and satisfaction.
Act as a resource to the organization for breast health navigation.
Establishes and maintains strong working relationships with providers, patients, and care givers.
Adhere to all company policies and procedures.

Registered Nurse (RN) or ARNP, graduate of an accredited school of professional nursing.
Valid Florida RN license.
Current BLS certification required, ACLS preferred.
Minimum 3-5 years’ experience in breast health; oncology experience a plus.
Must possess strong customer service and communication skills.
Must possess characteristics of self-motivation, initiative and flexibility in response to changing organizational needs.
Ability to easily navigate EHR systems and various other electronic documentation platforms.
eCW electronic medical records system experience
Certified Breast Care Nurse (CBCN) highly desired
